A sample UCT Transition Planning Questionnaire can be used to gather the information needed to create a Transition Plan. This questionnaire can be used as is or can be amended to your purposes. The questionnaire may also be used as an interview guide for a one-on-one discussion (the discussion does however need to be documented).

Note:
-    A Transition Planning Questionnaire is different and separate to an Exit Interview. A Transition Planning Questionnaire focuses on passing on institutional knowledge to benefit future employees and is position related. An Exit Interview gathers feedback to improve organisational practices, based on the lived experience of the staff member specifically.
